New Patient Registration

Patients are registered with valid evidence of proof of address and passport. All documents must be originals.

If we are unable to accept you on our list or when you are removed from our list, we will advise you regarding a local practice that may be able to accept you.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Details

It is important for you that we know how to contact you, please advise us as soon as there are any changes to your details such as a telephone number and address.

If you move house a short distance outside the area covered by the practice, you may have to register with another practice. Please discuss with us.
